eCW Medical History Form
If you are an eCW customer using NexHealth Forms, you have access to the integrated Medical History Form, which automatically syncs patients’ Allergies, Conditions and Medications to the upcoming Progress Note in eCW.
Here’s how it works:
- The NexHealth Medical History Form is already included in your account. It features search-and-select free-text questions for patients’ Allergies, Conditions and Medications.
- After you send the Medical History Form, the patient types in their corresponding health history, which automatically pulls matching options from eCW.
- The patient’s Allergies, Conditions and Medications are automatically synced to the Progress Note field in the patient file in eCW. The PDF forms are synced to Patient Docs.
Get started:
You can start sending the Medical History Form to patients today. Here’s how:
- Navigate to the Forms tab in NexHealth and select Manage forms.
- Use the search bar at the top or scroll down the page, to locate the Medical History Form and ensure that it is active in your account. If you want to send the Medical History Form right away, go ahead and create a Form Request!
- To add the Medical History Form to a packet, switch to the Packets tab.
- Select the pencil icon to edit any of your packets.
Medical History Form FAQ
Q: Will any new locations that I add also have the Medical History Form?
A: It will automatically generate when the Synchronizer is installed.
Q: What if my patient doesn’t have a future appointment and therefore Progress Note?
A: The Medical History Form will sync to Patient Docs like normal, but there will be no area for Allergies, Conditions or Medications to write to, so they will not automatically appear if there is no Progress Note available.
Q: Can I add more questions to the Medical History Form?
A: No problem! Our Support team is happy to help augment the form, but please keep in mind that additional questions will not sync to the health chart like Allergies, Conditions and Medications do.
